STA GOLF TOURNAMENT SIGN-UP SHEET
Ladies and Men's Golf Scramble

We have scheduled a shotgun golf outing for Wednesday, October 19 at 12:30 p.m. at Atlantis Country Club.
Tournament fee is $60.00. Club rentals are available at the pro shop ($55.00). There is no shoe rental. Box lunches will be provided to all golfers, courtesy of the STA.
We have blocked a limited number of reservations for this event and we ask that you register with us no later than October 10th to insure your entry. Any cancellations received after that date will still require the payment for tournament fees.
We hope that you will take this opportunity to join us for a relaxing afternoon prior to the start of our exciting business program.
